29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a. On or about May 13, 2022, at the facility, employees were changing-out rotary die cutting boards on a rotary die cutter machine, during which time Lock-out or Tag-out devices were not affixed by the authorized employees to the energy isolating device for the Die Cut Section of the machine; therefore, exposing the employees to amputation hazards.
