29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (b)Forest Products Distributors, Inc. at 5406 Old Folsom Road, Rapid City, SD 57703: On or about and at times prior to May 12, 2022, the employer did not ensure that manufacturer installed guarding of moving parts presenting caught-in, crush-by or amputation hazards were functional and complete on Pellet Mill 2, in that holes were cut into the guarding. This condition exposed employees to laceration and amputation hazards.
