29 CFR 1910.119(i)(2)(ii):The pre-startup safety review did not confirm that prior to the introduction of highly hazardous chemicals to a process the safety, operating, maintenance, and emergency procedures were in place and were adequate: a) Plant; on or about April 25, 2022, employees were exposed to anhydrous ammonia hazards, in that the employer did not ensure that the pre-startup safety review confirmed that the safety, operating, maintenance, and emergency procedures were in place and were adequate to prevent the rupture of the 16 in. diameter suction line due to hydraulic shock.
