29 CFR 1910.269(d)(6)(vii): Before starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergizing of the machine or equipment had been accomplished: (a) Manchester Street Station - Generator #10: On or about 4/21/2022, the authorized employees did not adequately perform absence of voltage verification prior to employees returning to work after lunch, exposing employees to electrical shock hazards.
