29 CFR 1910.305(c)(3)(i): Single-throw knife switches and switches with butt contacts shall be connected so that the blades are deenergized when the switch is in the open position. On or about March 29, 2022, and times prior thereto, Baler Work Area, the baler machine, contained a non-functional butt contact switch, exposing employees to amputation and pinch-point hazards during baling operations.
