29 CFR 1910.147(d)(4)(i):Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) On or about 3/9/2021, and at times prior, at 110 Plugs Drive, Decatur, AL 35601, on Line 49, employees working to thread/clear jam of sealing substrate on VisionPak employees were exposed to unguarded moving machine parts which could cause contusions, fractures, or disfigurement including amputation because neither lockout nor tagout devices were affixed to each energy isolating device by authorized employees prior to accessing the point of operation.
