29 CFR 1910.147(a)(2)(ii)(B): Lockout/tagout procedures were not implemented when servicing and/or maintenance was taking place during normal production operations when an employee was required to place any part of his/her body into area on a machine or piece of equipment's point of operation. a. 200 S. First St., Elizabeth, NJ - Wire recycling area The employer did not ensure that Lockout/Tagout procedures were used when performing blade changes on the Genox Machine. Employees engaged in the recycling metal materials performed blade changes on the Genox grinding machine where the cutting blades were required to be changed regularly and the equipment was not locked out when the change was being completed. Violation occurred on or about February 18, 2022
