29 CFR 1910.217(c)(1)(i): It shall be the responsibility of the employer to provide and insure the usage of "point of operation guards" or properly applied and adjusted point of operation devices on every operation performed on a mechanical power press. See Table O-10. a) On or about February 11, 2022, the employer had not established a procedure to be followed by employees when working with the die. During a die set up, a press operator had their hands in and near the die without any point of operation guarding, and no safety procedures in use to protect the employee from gravitational energy from the upper die in that the interlocked safety block was not used.
