OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. In the Main Building, employee(s) engaged in a new custom order process working from a platform were exposed to crush by hazards that were previously identified in a previously conducted risk management assessment but was never communicated to employee(s), nor was training provided to the affected employee(s) before conducting this work, resulting in employee injuries requiring hospitalizations.
