Failure to establish written mechanical integrity inspection and test procedures can lead to flammable liquids being released and employee exposure to fire or explosion hazards, on or about February 2, 2022. b) Polymerization Plant 2: The employer did not establish written pressure vessel and piping mechanical integrity inspection and test procedures for de-volitizers (D1-2).
