29 CFR 1910.29(g)(1): The employer did not ensure that cages and wells installed on fixed ladders are designed, constructed, and maintained to permit easy access to, and egress from, the ladder that they enclose (see Figures D-14 and D-15 of this section). a) Processing Area, Exterior: On December 6, 2021, and at times prior, the employer exposed employees to fall hazards of approximately 6 feet in height, in that; the cages around the fixed ladders on the star screen had warped and damaged hoops and verticals, impeding easy access and egress as employees used the ladders to access the platforms around the machine.
