29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: General Maintenance Department: On or about December 06, 2022, the employer did not establish a program consisting of energy control procedures, training, and periodic inspections to ensure that employees responsible for mechanical maintenance operations were protected from injury by the unexpected energization of the machines and/or equipment while it was being serviced. Employees performed servicing and maintenance on equipment that included, but were not limited to, a 120V DW756 bench grinder and a 120V metal band saw (serial number: 181020283).
