29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): a) La Jolla Indian Campground Zip Line: On and before October 30, 2021 employee(s) rode in the back of a pickup truck standing to the rear of the truck bed leaning against a barrier chain. A hazard assessment was not conducted to determine the hazards present and the necessary personal protective equipment (PPE) for each hazard. Employee(s) were exposed to ejection and/or fall hazard. b) La Jolla Indian Campground, Zip Line Retrieval and Launching Platforms/Areas: On and before October 30, 2021, Employee(s) worked on platforms/areas recovering and launching customers. A hazard assessment and equipment selection was not conducted to determine the hazards present in the workplace and the necessary personal protective equipment (PPE) for each hazard. Employees were exposed to fall and struck by hazards.
