29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about October 19, 2021 at worksite located at 5431 Larchwood Lane, Toledo, OH: The employer, Jorge Medina, did not ensure that the portable ladders used by employee to access the roof in order to perform residential roofing work was extended 3 feet above the surface of the roof, exposing employees to falls from heights of approximately 8-24 feet to the ground below.
