29 CFR 1910.178(l)(4)(ii)(A):Refresher training in relevant topics was not provided to the operator that has been observed to operate the vehicle in an unsafe manner: a) On or about 09/24/2021 and at times prior to, at the dumpster on site, employees were elevated on pallets with a forklift resulting in fall hazards. Refresher training was not provided to employees operating the forklift in this unsafe manner. b) On or about 10/01/2021, near the weld shop, employees operated the forklift with propane cylinders not secure to the forklift. Refresher training was not provided to employees operating the forklift in this unsafe manner.
