29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: C & D Cup Robot area: On or about September 29, 2021, machine guarding methods installed by the employer to protect employees with interlocks that immediately stop machine motion, were not provided when the interlock switch of the cage barrier failed due to improper wiring that was not tied into the safety device to adequately disable the machine from energizing when it was engaged. This resulted in a struck-by injury from the palletizing robot arm when the door of the guarding system was opened, and the machine was still in motion.
