29 CFR 1910.147(d)(5)(i):Following the application of lockout or tagout devices to energy isolating devices, all potentially hazardous stored or residual energy were not relieved, disconnected, restrained, and otherwise rendered safe. a) Omnisource LLC is located at 1500 Old Bowman Street Road Mansfield, OH, 44903: On or about September 9, 2021, the employer failed to ensure that all potentially hazardous stored or residual energy was relieved, disconnected, restrained, and otherwise rendered safe. An employee was working on welding a baler ram of a Harris Baler Model TG 2102 where a lockout pin was removed resulting in the ram falling approximately 8 inches hitting the employee in the head and shoulder.
