29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) At the facility, on or about June 23, 2021, the employer did not ensure the use of lockout tagout procedures by employees who performed maintenance work on various valves and performed line clearing on the transfer lines between tanks, thus exposing employees to chemical hazards.
