29 CFR 1910.146(k)(3)(i): Each authorized entrant did not use a chest or full body harness, with a retrieval line attached at the center of the entrant's back near shoulder level, above the entrant's head, or at another point which the employer can establish presents a profile small enough for the successful removal of the entrant: On or about May 13, 2021, and at times prior, at the Kerr Dam, employees were not provided and did not use non-entry retrieval equipment for the successful removal of the entrant when they entered into the inspection gallery to perform work and were exposed to hazardous conditions. Pursuant to 29 CFR 1903.19, within thirty (30) calendar days of the date of this citation, the employer must submit documentation describing the steps taken to ensure each authorized employee who enters a permit-required confined space use a chest or full body harness, with a retrieval line attached at the center of the entrant's back near shoulder level, above the entrant's head, or at another point which the employer can establish presents a profile small enough for the successful removal of the employee.
