29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Pit Mounted Centrifugal Casting Machine "A Spinner": On or about April 26, 2021, the employer did not ensure employees involved in the spinner process preparation while working on and around the centrifugal casting machine were protected from exposure to the rotating spinner die and attached parts. Employees were exposed to a caught on hazard from the spinning die while standing in the centrifugal casting pit to clean and apply a release compound to the die prior to casting operations. (b) Pit Mounted Centrifugal Casting Machine "B Spinner": On or about April 26, 2021, the employer did not ensure employees involved in the spinner process preparation while working on and around the centrifugal casting machine were protected from exposure to the rotating spinner die and attached parts. Employees were exposed to a caught on hazard from the spinning die while standing in the centrifugal casting pit to clean and apply a release compound to the die prior to casting operations. (b) Pit Mounted Centrifugal Casting Machine "A Spinner": On or about August 03, 2021, the employer did not ensure that an employee working within the spinner die to conduct die cleaning and application of a release compound were protected from inadvertent start-up of the spinning machine. This exposed employees to rotating parts, caught-on, ingoing nip points and crushing hazards.
