29 CFR 1910.134(k)(1): The employer did not provide effective training that covered the required elements in 1910.134(k)(1)(I) - (vii): Physicians Ambulance Service Inc. located at 4495 Cranwood Parkway Cleveland OH 44128: On or about April 23, 2021, and continuing thereafter, the employer did not provide employees with effective training on why the respirator is necessary and how improper fit, usage, or maintenance can compromise the protective effect of the respirator; what the limitations and capabilities of the respirator are; how to use the respirator effectively in emergency situations, including situations in which the respirator malfunctions; how to recognize medical signs and symptoms that may limit or prevent the effective use of respirators; and the general requirements of this section. The employer required employees to wear N95 filtering facepiece respirators to protect against the SARS-CoV-2 virus while providing care to suspected and confirmed positive COVID-19 patients.
