29 CFR 1910.178(l)(1): The employer did not ensure that each power industrial truck operator was competent to operate a power industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in paragraph (1): a. 100 Middlesex Ave., Suite 2, Carteret, NJ - Warehouse The employer did not ensure that employees operating powered industrial trucks were provided training. An employee engaged in warehousing activities operated a Toyota forklift that was not authorized, trained, and evaluated by the employer to operate the equipment, resulting in an accident that caused injury to the employee.
