29 CFR 1926.453(b)(2)(v):A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: On or about March 05, 2021, on the northwest side of the project site, an employee was utilizing a JGL 660SJ Telescopic Boom Lift (SN#0300276989) without the use of a body belt as a tethering / restraint system while performing masonry work on the exterior of a building at heights upwards of ten (10) feet.
