29 CFR 1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a. 24-36 Fadem Rd., Springfield, NJ - Production Area The employer did not ensure that machines were guarded in a manner to prevent employees from having any part of their body in the danger zone. Employees engaged in the manufacture of window frames used a Sampson, Model SW600, S/N: 95190, sash welder, where the floor mat anti-activation pad had wires severed and was not connected to the machine in a manner that would prevent activation of the equipment when the employees were placing frame pieces into the machine. Violation observed on or about February 4, 2021
