29 CFR 1910.119(l)(1):The employer did not implement procedures to manage changes to process chemicals, technology, equipment, and procedures, and changes to facilities that affect a covered process: On or about April 29, 2021, where a Dunham-Bush shell and tube heat exchanger was replaced with a Kelvion plate and frame brazed heat exchanger, the employer did not follow established management of change procedures in that it did not initiate a management of change record.
