29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck At a facility located at 2221 North Belcrest, Springfield, MO 65806: The employer failed to protect employees from struck-by and crushing hazards. This was most recently demonstrated on or about May 26, 2021 and at times prior there to when employees were permitted to operate powered industrial trucks in the facility and they had not been properly trained in the safe operation of the powered industrial truck.
