29 CFR 1910.147(c)(5)(ii)(D): Lockout devices and tagout devices shall indicate the identity of the employee applying the device(s): Employee(s) engaged in an ammonia line break to replace barrel tubes for the Thermutators in the Taco room, located at 512 S. Main Street, Hutchinson Kansas were exposed to burn / inhalation hazards due to the lockout and tagout devices not identifying the team members who attached the lockout devices.
