29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a. On or about October 15, 2020, and at times prior, authorized employees who performed regular maintenance and servicing activities, were not provided with training on hazardous energy control. Employees performed maintenance and servicing on equipment such as, but not limited to, Kilns (7 total), Mixers, Stokes R4 Part-Revolution Mechanical Power Press (MPP), Wash Tanks, etc. As a result, employees were exposed to hazardous energy sources such as, but not limited to, electrical, gravitational, hydraulic, pneumatic and thermal. b. On or about October 15, 2020, and at times prior, in the Press Department, an employee performed cleaning of the dies on the Stokes R4 Part-Revolution Mechanical Power Press (MPP), without being provided with training on hazardous energy control. As a result, the employee was exposed to hazardous energy sources such as, but not limited to, electrical and gravitational.
