Text For Citation: 01 Item/Group: 004 Hazard:
29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a.) 6804 S. Ensenada Street, Aurora CO 80016: On or about October 5, 2020, and at times prior, the employer, DVJ Concrete and Caisson Drilling, did not enforce use of protective helmets when there was a possible danger of head injury from impact or falling objects. On October 5, 2020, two employees were working in the fall zone of a 30 foot long steel pipe casing that was lifted in the air approximately 10 feet high. The employees were not wearing hard hats. The pipe came loose from the hoist hook and fell to the ground, striking both employees in the head after it bounced on the ground.
