29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Standard Pipe Services, LLC temporary site located at 111 Forge Ct, Malvern, PA 19355: The employer did not provide a ladder that reached at least 3 feet above the pavement for employees entering and exiting a storm sewer pipe approximately 6 feet 7.5 inches underground, thus exposing employees to a fall hazard. The condition was noted on or about September 17, 2020.
