29 CFR 1910.147(e)(1): Before lockout tagout devices were removed, the work area was not inspected to ensure that nonessential items had been removed or ensured that machine or equipment components were operationally intact. a) At the facility, on or about September 3, 2020, lockout tagout devices on dock plate #3 were removed without ensuring the machine or equipment components were operationally intact, thereby exposing employees to crushing, struck-by, and caught-in hazards.
