29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): On or about August, 28, 2020, at the location of 3225 W. 71st St., CLEVELAND, OH, 44102, the employer did not ensure that each powered industrial truck operator was competent to operate forklift trucks in the facility, including, but limited to a Yale pallet jack (Serial Number N56001-3) as demonstrated by the required training and evaluation.
