29 CFR 1910.252(b)(4)(iv): Where the welder must enter a confined space through a manhole or other small opening, means was not provided for quickly removing the welder in case of emergency. An attendant with a preplanned rescue procedure was not stationed outside a confined space opening to observe the welder at all times in case of emergency a) On or about July 14, 2020 Chicago Boiler Company dba CB Mills Division of Chicago Boiler Company, did not ensure means were provided to quickly remove the welder when employee(s) performed welding in confined spaces and did not ensure that an attendant with a preplanned rescue procedure was provided outside the confined space to observe the welding. In accordance with 29 CFR 1903.19 (d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstration that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of purchase or repair of equipment, photographic or video evidence of abatement or other written records.
