29 CFR 1910.146(d)(9): The employer did not develop and implement procedures for summoning rescue and emergency services, for rescuing entrants from permit spaces, for providing necessary emergency services to rescued employees, and/or for preventing unauthorized entry. a) Wash bay: On July 3, 2020, employees entered a permit space of a semi-trailer tanker attempting a rescue of an employee where the employer did not have procedures to prevent unauthorized entry.
