29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: On or about 6/30/2020, at a worksite located at SW 160th Avenue in Miramar, FL 33023, the employer did not ensure the use of a hard hat by an employee who was working in the immediate area where palm fronds were being cut, exposing employee to a struck by hazard.
