29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: At 4115 S. Johnson Rd, Chanute, KS in the maintenance department, Employees engaged in maintenance activities were exposed to hazardous energy, such as 480VAC and pneumatic power in that locks were not utilized nor provided for the locking out of equipment, increasing the potential for maintenance employees to be exposed to electrical hazards, amputation, caught-in, and struck by hazards.
