29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting: On or about 3/17/2020 on the production floor, operators working at or walking by the production lines including but not limited to line two were exposed to caught in and amputation hazards where the employer failed to ensure that all horizontal rotating shafts were protected at: a. side stuffer - four-inch, keyed shaft, four inch wide X ten inch tall opening, 48-inches above the walking working surface; b. side stuffer - four-inch, keyed shaft, four inch wide X ten inch tall opening, 48-inches above the walking working surface; c. rotor (SCC ACC #0156, 8" SLCM) - fully exposed four-inch, keyed shaft, 41-inches above the walking working surface; d. rotor (SCC ACC #0156, 8" SLCM) - fully exposed four-inch, keyed shaft, 50-inches above the walking working surface; e. WEG gearbox - two-inch, keyed rotating shaft - two inch opening, 48-inches above the walking working surface; f. WEG gearbox - four-inch, keyed rotating shaft - four inch opening, 48-inches above the walking working surface; g. four inch, keyed rotating shaft, seven and one half inch opening, 48-inches above the walking working surface. h. vent stuffer, four-inch unguarded vertical shaft was 75-inches (six-foot three-inches) off of the walking working surface.
