29 CFR 1910.119(e)(3)(vi): The process hazard analysis did not address human factors: a) Delaware City Refining Company, LLC (Desulfurizing Unit) - On or about March 11, 2020, the employers process hazard analysis failed to address human factors relating to Control Room Distributed Control System (DCS) user-machine interface as operators were overwhelmed by the number of alarms and critical safety alarms were not easily distinguishable from upset alarms during a heater tube rupture in heater 29-H-4. The resulting fire caused two operators to sustain serious burn injuries.
