29 CFR 1910.304(b)(3)(ii)(A): All 125-volt, single-phase, 15, 20 and 30-ampere receptacle outlets that are not part of the permanent wiring of the building or structure and that are in use by personnel did not have ground-fault circuit-interrupter protection for personnel: a) Serco Chesapeake Facility: On about 02/04/2020, an employee connected two (2) extension cords together then plugged the male end of the extension cord into a wall mounted receptacle outlet to supply power to a portable grinder, vacuum cleaner and pedestal fan without ground fault circuit interrupter protection. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
