29 CFR 1910.179(j)(2)(iii): Monthly inspections of hooks, with a certification record which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the hook inspected, were not performed: The employer is failing to protect employees from struck-by and caught under hazards associated with using hooks to lift and move loads. The most recent occurrence was evident during the inspection conducted on January 28, 2020 at the facility located at 4608 S. 25th St, Norfolk, NE, 68701. The employer has employees lifting loads using load block hooks that have not had a monthly inspection with a certification record.
