29 CFR 1910.217(f)(2): The employer did not train and instruct the operator in the safe method of work before starting work on any operation covered by this section and the employer did not insure by adequate supervision that correct operating procedures were being followed: a) Job site, 33 McGovern Road, Crescent, PA: On or about November 23, 2019, during production activities on the Torc-Pac part revolution press number 11, serial number 57-7135 and Warco part revolution press number 2, serial number 5346, both located in building 4, the employer did not ensure that employees were properly trained in press safety or that proper safety practices were followed while operating these machines. Deficiencies noted during the inspection included, but were not limited to, not ensuring that the staff understood at least mechanical power press safety basics such as the relationship between 2 hand controls and stop times for machine guarding, that ghost/double cycles were not permissible or that mode selector switches needed to be supervised and failing to properly enforce safety practices regarding not reaching into press danger areas.
