29 CFR 1926.251(a)(1): Rigging equipment for material handling was not inspected prior to use on each shift and as necessary during its use to ensure that it was safe: Building7, 285 Locust St., Woburn, MA On or about October 16, 2019, workers were exposed to falling suspended loads being hoisted by the forks of a powered industrial truck with an automotive tow strap that was not designed, proof-tested prior to use to 125 percent of their rated load. The rated capacities by sling configuration (i.e., working load limits) were not indicated on the sling by permanently affixed and legible identification markings prescribed by the manufacturer.
