29 CFR 1910.119(i)(1): The employer did not perform a pre-startup safety review for new facilities and for modified facilities when the modification was significant enough to require a change in the process safety information. At the worksite located at 2504 Industrial Drive, Kirksville, Missouri: Employees were exposed to hazards of ammonia exposure when a release occurred on October 2, 2019, and October 3, 2019. The facility had undergone an expansion that added an additional ammonia system. The employer did not perform a pre-startup safety review prior to putting the expansion into production.
