County Road T - Fremont, Nebraska - 68025 - The employer is failing to provide effective respiratory protection training before employee(s) use respirator(s) in the workplace including, but not limited to respirator fit, respirator limitations, proper maintenance, and medical signs/symptoms that may limit usage. Employee(s) were required to wear full facepiece tight-fitting cartridge respirator(s) during the monitoring and opening of shipping containers treated with phosphine gas. This violation last occurred on or about August 31, 2019.
