29 CFR 1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices: On or about September 12, 2019, the employer failed to assess all the potential energies were relieved, restrained or rendered safe. The employee(s) were in the process of heating an R-22 freon joint near the expansion valve on the #5 chiller. This line still contain residual amounts of R-22 freon and an oil mixture. Upon heating up the solder on the joint a flash fire occurred. Employee(s) were exposed to a burn hazard.
