29 CFR 1910.147(c)(7)(i)(A): Authorized employees did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a) At the facility, building 5 rail transfer area: On or about August 31, 2019 employees conducting cleaning of transfer system hose and piping were not trained in the recognition and means of isolating automatic closing pneumatic gate valves, exposing employees to an amputation hazard.
