29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary: (a) On or about 7/23/2019 - 4901 Twin Pine Circle, Hoover, AL, employees were exposed to cave-in and struck-by hazards when working in a trench and the spoil pile was located at both edges of the trench. The trench measured approximately 5 to 7 feet deep, 70 feet long, and 3.5 to 5 feet wide with spoil piles up to 5 feet high located on either side and at the edge of the trench.
