At the facility, on or about 9/5/2019: Employees performing maintenance and/or repairs on equipment, including, but not limited to washers, dryers, shuttles and linen folders, did not receive adequate training in the application and understanding of lockout procedures and the knowledge and skills for the safe application, usage and removal of the energy control devices.
