29 CFR 1910.146(c)(7)(iii): The employer did not document the basis for determining that all hazards in a permit space had been eliminated, through a written certification that contained the date, the location of the space, and the signature of the person making the determination: (a) PRODUCTION AREA - Employees performed cleaning operations inside of pieces of equipment identified as aspirators, which are classified as permit required confined spaces. Employees who entered these spaces were not required to obtain any kind of permit to verify that the physical hazards from the 12 inch auger and the rotating cage fan had been controlled, prior to entering the spaces, on or about August 15, 2019. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
