29 CFR 1926.959(d)(2): A designated employee other than the equipment operator did not observe the approach distance to exposed lines and equipment and provide timely warnings such that the minimum approach distance was being maintained before the minimum approach distance required by paragraph (d)(1) of this section was reached. At a worksite located at 6760 West Farm Road 68, Willard, Missouri 65781, employees were exposed to serious electrical shock and burn hazards. A designated employee was not utilized to observe the minimum approach distance of two employees in a utility bucket truck to exposed lines and equipment and provide timely warnings such that the minimum approach distance was being maintained before the minimum approach distance required by paragraph (d)(1) of this section was reached. Employees were exposed to 7.62kV of electrical current.
